Boots Health Club
By filling in this application form you confirm that you would like to be a member of the Boots Health Club and you're happy for us to communicate with you by post, email and by text message. We'll also send you exclusive offers as part of our general Advantage Card programme.
By providing your email address you'll be automatically registered with boots.com. You'll receive email communications from boots.com and the Boots Health Club.
If you're already an Advantage Card holder and have previously opted not to receive mailings, upon joining the Boots Health Club you'll automatically receive mailings from both Boots Health Club and Advantage Card.
In order to be a member of the Boots Health Club, you must have an Advantage Card. If you don't already have one, you'll be signed up for the Advantage Card scheme and sent a Permanent Advantage Card as part of joining the Boots Health Club and will automatically receive Advantage Card mailings.
Boots Health Club is open to all UK residents aged 18 or over - the Club isn't available to customers in the Republic of Ireland Advantage Card scheme.

Data Protection: We'll never give your personal details to anyone outside the Boots Group of Companies and their successors. Unfortunately if you do not wish to give us your details on this form or do not want the Boots Health Club to communicate with you by post you will be unable to join.
We can take any action we consider appropriate if we have reason to believe that you are abusing the Boots Health Club including withdrawing your club membership and over 60's discount where applicable.

Boots Health Club topics
As a Boots Health Club member you can't opt in to more than three topics at any time. If you're already signed up to three topics you will need to choose a topic to delete before you add another. You can amend your topic choices by calling us on 0845 124 4545 (BT users are charged a lo-call rate. Other network rates may vary, please refer to your service provider for details. We may record your call to help us maintain high standards of service to you).
You can amend topic choices but this will only take effect the next time we send out mailings, usually every six months.
We may introduce new topics, and get in touch with you about these if we believe they may be of interest to you.
You'll only receive one welcome pack, when you initially join the Boots Health Club. You won't receive any further welcome packs when you sign up to new topics at a later date.

Advantage Card Scheme

All Advantage Cards belong to Boots.
The UK Advantage Card Scheme is valid in all UK Boots stores; it is not valid in Republic of Ireland stores.
Your Advantage Card is personal to you. We do not issue additional cards for your account but your family and friends can of course apply for their own cards

Collecting points in Boots stores
You will receive points on qualifying purchases at the rate notified from time to time by the display of notices in Boots stores and on www.boots.com. When you present your Advantage Card, in return you will receive not only the goods and services you have chosen, but Advantage Card points as well. The number of points issued may vary between qualifying purchases. We will not exchange Advantage Card points for cash.
Points will usually be added to a valid Advantage Card at the time you buy products and services included in the scheme. There may be specific occasions when we add points at a later date at our discretion, however you will need to provide a validated receipt within 30 days of making the purchase. If you forget your Advantage Card please ask the cashier to validate your receipt.
We might change the number of points you collect on products and services included in the scheme. We'll let you know with notices in Boots stores and give details at www.boots.com. At Boots Opticians you'll collect 1 point for every £1 spent.
In-store and online points are not available on prescription items, infant milks (up to 6 months), gift vouchers/cards or postage stamps.
During triple points events both items in-store and on-line which are already included in additional, double, or triple points promotions are excluded.
Collecting points on www.boots.com
The number of points collected through third party websites can vary from 4 points per £1, please check individual sites for details.
Points promotions, such as Triple points may also vary for third party websites (other Shops) and Boots Services website. Check individual promotions for exclusions and individual websites for additional points offers, which are offered periodically.
Points are not available on gift boxes and delivery charges.
Spending Advantage Card points
We're sorry but points cannot be spent in Boots Opticians or on www.boots.com. In store points can't be spent on prescription items, stamps, gift vouchers/cards, infant milks (up to 6 months), mobile phone top-up cards and insurance products. Please look for the * on the price ticket as this tells you that you can pay with points.
You cannot make a part payment of points towards a product or service. You must collect the full amount of points needed for the product you choose. These will be deducted from the total on your card.
You won't qualify for points or extra point offers on products or services, which you pay for with points.
We can substitute or remove products and services in store and other third party products and services available for points collection and redemption.
To spend your points simply select any product with an * on the price ticket in-store. Take the item to the till along with your Permanent Advantage Card and let the cashier know that you'd like to pay with your points. The total number of points will be deducted from your Card.
Returning goods
If you decide to return a product, which you bought with cash, card or cheque, we'll deduct the points from your Advantage Card that you collected when you brought the product.
If a product you buy with points is faulty, please return it to any Boots store. If necessary we will re-credit your account with the right number of points. No cash refund will be given in these circumstances.
When returning goods from www.boots.com unless you are requesting an exact replacement for your return, any Advantage Card points collected on items will be deducted from your Card.
The NHS Organ Donor Scheme
Boots organ donor registration is run in partnership with the NHS.
We're happy facilitate the scheme, passing relevant information on to the NHS to be included in the Organ Donor Register from time to time.
You are free to remove the Organ Donor symbol from your card by contacting the helpline on 0845 124 4545. If you let Boots know that your details have changed, please also let the NHS Organ Donor Scheme know.
If you would like to register specific organs for donation, please call the NHS Organ Donor Line on 0845 60 60 400 or visit the website at www.uktransplant.org.uk
If you have previously registered specific organs for donation, your wishes will not be overridden by completing this application form.
We will print the NHS Organ Donor symbol on your card and forward your name, address, sex and date of birth to UK Transplant for inclusion on the NHS Organ Donor Register. If at any time you change your mind, just call the Advantage Card help line on 0845 124 4545.
